How can health care be enabled to develop and deliver effective tailored care? And what role does science play in this? Both Carla Nouwen and Marcel van Eck, from the Academic Collaborative Center Social Work, are conducting research into how integrated care pathways can be designed. Both researchers have produced interesting publications this year and these have led, among other things, to a broadcast in the NPO documentary Verloren hulpverleners(new window).

Knowledge about how to design care pathways in an integrated manner is lacking among social workers, informal caregivers, managers, policymakers, and grant providers. There is increasing demand for collaboration among the various social workers involved in complex issues. Therefore, the studies contribute to answering the question of how the various actors in care and well-being can be empowered to develop and deliver effective tailored care.

Among other things, Carla Nouwens looked at how integrated care can prevent different social services from working in isolation. The research shows that an integrated approach has several benefits for clients, including increased co-ownership, self-regulation, and inner peace.
